Southern Chiropractic offers our patient forms online so they can be completed in the convenience of your own home or office. To do so, download, complete, and bring the forms with you to your appointment.

Every new patient will need to complete a Patient Intake Form. Patient Intake Form let us know the history and current state of your health. This includes a past history form. In addition, patient will need to complete an appropriate form related to their complaint(s).

What to Expect

The Foundation

Chiropractic care is a step-by-step process. Healing takes time and occurs in a particular order in order for your body to be strong and work correctly. Healing starts with proper joint function in all direction of movement. The integrity and balance of the muscle, fascia, ligament, tendon, and joint complex are keys to proper healthy biomechanical joint function and locomotion. Abnormal joint function creates stress and pressure on joints and over time causes the joint to lose its integrity leading to injury and pain. Chiropractic care helps correct abnormal joint function and allow your body to repair itself correctly and fully.

What to expect on your first visit

A chiropractic visit is similar to other healthcare appointments. After you complete the necessary paperwork, your chiropractor will discuss your symptoms with you and perform a thorough examination, which may include:

Phases of Care

Phase 1: Relief Care

This is the first phase of care when you are in pain. The first objective is to correct abnormal joint funtion so your body can start the healing process. Therapeutic therapy is often use in this phase of care to help reduce inflammation or help reduce muscle tension/spasm. Depending on the severity of your problem and nutritional status, this phase can take several weeks.

Phase 2: Corrective Care

The corrective phase allows your joints, muscles, ligaments, and tendon to heal more completely. Soft tissue works and/or rehabilitation exercises with chiropractic adjustments are often use to stabilize the joints, strengthen the surrounding soft tissues, and correct other soft tissue abnormality to built a strong foundation and enhances further healing. This phase can take a few more weeks to a few months, depending on the severity of you problem and your overall health.

Phase 3: Wellness Care

Once your body has fully recovered, it is important to come in for periodic chiropractic adjustments to avoid problems in the future. This can be monthly to several times per year, depending on your activity levels and goals.
